A 40-year-old man in Hsinchu suffered a rare but serious injury while celebrating his wedding anniversary with his wife. During an intimate moment, he experienced such intense trauma that his penis fractured, causing excruciating pain that left him unable to stand. He was rushed to Tung-Yuan General Hospital’s emergency department, where urologist Dr. Dai Shun-Ching diagnosed a rupture of the penile corpora cavernosa, with blood pooling in a “gooseneck” formation. The medical team immediately performed an urgent penile tunica albuginea repair surgery, and the patient successfully regained full sexual function post-operation.

Dr. Dai explained that although the penis has no bone, it consists of two corpora cavernosa and one corpus spongiosum surrounding the urethra. During an erection, the corpora fill with blood, but the outer tunica albuginea is relatively thin and vulnerable. Sudden, forceful impact can easily cause it to rupture.

Clinical data show that around 80% of penile fractures result from sexual activity, most commonly in the “woman-on-top” position. If the penis slips and twists during intercourse, the tunica can tear. Injuries are often accompanied by a loud “pop,” severe pain, swelling, deformity, and sometimes subcutaneous bruising with deep purple discoloration.

For minor injuries, oral medication may suffice, but severe fractures require surgical repair. Surgery is performed under partial anesthesia: the surgeon stops the bleeding, removes pooled blood, sutures the torn tunica albuginea, and injects saline to ensure the repair is complete. Patients are advised to abstain from sexual activity for around six weeks to allow full recovery.

Dr. Dai emphasized that immediate first aid is crucial. Applying a compressive bandage and seeking medical care within six hours provides the best chance for full recovery. Delays can lead to permanent complications such as penile curvature, arteriovenous fistulas, erectile dysfunction, or tissue necrosis. Severe injuries can even affect the urethra, causing blood in the urine or difficulty urinating.

He also urged couples to exercise caution during sexual activity: avoid excessive force, risky positions, and tight spaces, as a moment of passion can have long-lasting consequences.
