DOSH Issues Stop-Work Notice After Two Workers Killed In Selangor Construction Crane Accident

The Department of Occupational Safety and Health (DOSH) Selangor has issued a prohibition notice at a factory construction site following a fatal crane accident that claimed the lives of two foreign workers on July 28.

In a statement, DOSH Selangor said the notices were issued over several safety violations, including the failure to conduct a risk assessment for concrete lifting operations using a mobile crane, the absence of a proper Lifting Plan, and the failure to appoint a Site Safety Supervisor.

The accident occurred at about 5pm, when two general workers, aged 40 and 44, were killed after being crushed by the boom structure of a crane that toppled.

Preliminary investigations found the incident happened while workers were lowering a 400kg concrete bucket using a mobile crane.

DOSH said the crane is believed to have tipped over after its boom was extended beyond the recommended operating length.

Both workers were reportedly standing beneath the crane’s boom when the structure collapsed, trapping and crushing them.

They were pronounced dead at the scene after suffering severe head injuries caused by being pinned between the beam structure and the crane’s fly jib boom.

Their bodies were later taken to Hospital Banting for post-mortem examinations.

Following the incident, a team of investigators was dispatched to the site. Technical inspections revealed physical damage to the crane’s fly jib boom.

Further investigations also found that the construction site had not been registered with DOSH, while both the main contractor and subcontractor failed to produce any occupational safety and health management documents during inspections.

The case is being investigated under Section 15(1) of the Occupational Safety and Health Act 1994.
